Course of Interest
A $50.00 non-refundable application fee is due at the time of application for enrollment. Your application will not be processed until this fee is paid in full. Payment can be mailed with the complete registration package or via credit card. Please call the office at 801-772-3131 if you would like to pay with credit card.
Prospective students submitting an application are required to also submit copies of the following documents. Applications may be accepted prior to their receipt; however, students may not start classes without submission of all required documents.
- a copy of your drive'rs license, passport, state issued ID, or birth certificate.
- a copy of your social security card
- copy of your high-school diploma.
- general education diploma (G.E.D.), or ability to benefit exam(available through UVU).
Prospective Student Health
*Please be aware that full participation in esthetic or cosmetology treatments, both giving and receiving, is a requirement for satisfactory progress and course completion. Any condition that may prevent full participation must be disclosed fully prior to your enrollment. Please attach a physician's certification describing the condition and how it would affect your ability to fully participate. Without a physician's certification, failure to participate fully may adversely affect your grades and may even affect your ability to complete the course and graduate from Mandalyn Academy.

To the best of my knowledge, this application has been completed with information that is true and correct. I understand that supplying false information on any portion of this application may result in its rejection or, in expulsion from the Academy if information is determined to be false at any time during enrollment.
Before submitting your application, please review it to make sure that you have completed each section in its entirety and have attached all the required documentation (see following pages) as well as your non-refundable application fee. Incomplete applications or applications submitted without proper documentation and fees may not be processed.
Completed applications may be submitted by fax for the purpose of securing enrollment, however, all documentation is required prior to starting your program.
